How to request a replacement hearing aid
If you have lost your hearing aids or your hearing aids have been damaged or destroyed, you can request replacement hearing aids from Workcover.
There is a simple three step process:
1. Download the NSW Workcover "SIRA form 08022" Request for Replacement Hearing Aids.
2. Complete the Request for Replacement Hearing Aids Form.
